Gout Treatment and Prevention

While gout is usually a chronic and progressive disease, there are effective gout treatments. Gout treatment is two-pronged, concentrating first on relieving the pain and inflammation of an attack, and then on preventing future attacks and accompanying complications.

Painkillers, anti-inflammatories and corticosteroids target the pain and inflammation, while uric acid blockers or reducers are prescribed as preventatives. A prescription gout medication called colchicine can treat gout in two different ways, at higher doses to treat the pain and inflammation of an acute attack, or at lower doses to prevent recurring attacks.

Some general advice:

The first line of defense is to take a non-steroidal anti-inflammatory drug (NSAID) such as naproxen (Aleve), ibuprofen (Advil), indometacin (Indocin), or the newer celecoxib (Celebrex). These will ease most gout attacks in a day or two. If you have asthma, heart failure, high blood pressure or kidney problems, talk to your doctor before taking anti-inflammatories.

If anti-inflammatories don't work well enough, or you are unable to take them, your doctor will likely prescribe colchicine (Colcrys). Colchicine is an extract from a plant called the autumn crocus, also known as meadow saffron. While the persistent use of colchicine can reduce the attacks of gout, it does not prevent the accumulation of uric acid that can lead to joint damage even without attacks of hot, swollen joints.

Colchicine has been widely used in many countries since the 1930's, and was FDA approved in combination with probenicid (a gout medication which increases uric acid excretion) in the early 1980's. Colchicine was approved as a solitary gout treatment in 2009 under the brand name Colcrys.

Oral corticosteroids such as prednisone are also used to treat gout. Corticoseroids can also be injected directly into the affected joint to rapidly reduce inflammation and relieve pain. Corticosteroids are usually prescribed short-term because of potential side effects if used for a long time.

Allopurinal may be prescribed for chronic gout. Allopurinal is used to prevent, but not treat, gout attacks. It works by lowering the level of uric acid in the blood. Allopurinal must be taken daily, and may take 2 to 3 months to become effective.

It's important not to take allopurinal during a gout attack, but to wait until it has subsided. That's because it may cause an initial increase in uric acid levels, precipitating or worsening a gout attack.

Probenecid is another uric acid lowering medication that is sometimes prescribed in addition to low dose colchicine. Probenecid helps eliminate excess uric acid through the kidneys into the urine. It's important to drink lots of water when taking probenecid to prevent the formation of kidney stones. Some medications should not be taken along with probenecid, so make sure your doctor is aware of any other drugs you are taking.

Febuxostat is a newer preventative gout medication marketed as Uloric. It was developed specifically to treat gout by decreasing the body's formation of uric acid. It is suitable for patients with impaired kidney function.

Gout can't be cured, but it can be managed with lifestyle changes and gout medication. Are you doing all you can to manage your gout? Double check using this handy gout management checklist:

1. Avoid or limit your consumption of high purine foods such as red meat, fatty fish, yeast and legumes (peas, beans and lentils).

2. Avoid or limit alcoholic beverages, especially beer and wine, which are high in yeast.

3. Avoid or limit sugary drinks, including juice. One study found that two or more non-diet soft drinks a day increased the risk of gout by 85%.

4. Drink 6 to 8 glasses of water daily to help flush uric acid out of your system.

5. Eatlow-fat dairy products and fresh vegetables. Both have been shown to help prevent gout flare ups.

6. Maintain a healthy weight (gout is associated with obesity).

7. Experimentwith natural gout treatments such as apple cider vinegar, cherries, celery, omega 3 fish oils, and baking soda and to see what works for you.

8. Take a two-pronged approach to gout medications: anti-inflammatory drugs and painkillers to deal with attacks, and uric acid lowering drugs to prevent flare ups. Prescription colchicine is a frequently prescribed gout drug that both treats and prevents attacks.

9. Keep your blood pressure, cholesterol levels and blood sugar under control. High blood pressure and "bad" LDL cholesterol levels are associated with gout, as is diabetes.

10. Have your uric acid levels checked every few months to ensure they are under 6 mg/dL. (Note: Not everyone with high uric acid levels develops gout.)

Bananas as a Natural Gout Treatment

Christine Marquette, a registered dietician from the Austin Regional Clinic, has produced an informational video titled Nutrition & Diets: About Bananas & Gout. In the video, Marquette explains why bananas are thought to decrease gout symptoms.

She attributes bananas' ability to act as a natural gout treatment to their high levels of potassium and vitamin C. Marquette points out that gout is caused by high levels of uric acid in the body, and a gouty arthritis flare up increases the acidity of the body.

Potassium helps regulate the body's acid-base balance (the normal equilibrium between acids and alkalis). The potassium in bananas may decrease the acidity in the body, making it more alkaline.

As well, the vitamin C in bananas has anti-inflammatory qualities that may help reduce the painful inflammation which always accompanies a gout attack. While she doesn't go so far as to say bananas could replace gout medication, Marquette does feel the fruit should be a part of any gout patient's diet.

Arcalyst for Gout under Review by FDA

Regeneron Pharmaceuticals Inc. announced that FDA will review its drug Arcalyst as a treatment for gout. FDA plans to complete this review by July 30, 2012.

Arcalyst (rilonacept) is currently approved to treat Cryopyrin-Associated Periodic Syndromes (CAPS), a rare genetic anti-inflammatory disease. Regeneron reported $5.5 million in sales of the drug during the third quarter.

Gout Sufferers - Are your shoes helping you?

Researchers based out of AUT University in Auckland, New Zealand have discovered another environmental factor leading to gout flares- your footwear. "We found that gout patients in our study often wore improper footwear and experienced moderate to severe foot pain, impairment and disability," explains Professor Keith Rome, who led the research team.

Just over half of the subjects wore proper footwear, which includes walking shoes, athletic shoes ("sneakers") and loafers. All of these shoes have a solid foot bed, for good arch, ankle and knee support, and are close-toed. However, 42% of gout sufferers wore unsuitable shoes such as flip-flops, sandals and moccasins. Another factor of good shoes is their age. Some of the sufferers were wearing the right shoes, but researchers were finding them to be over 12 months old or older. The benefits of good shoes steadily decrease as the get older and more worn.

When choosing which shoes to wear or buy, consider these factors once you slip a pair on:

- Cushioning - Do you feel your feet lightly depress the insole? Do your toes have wiggle room?
- Support - Can you feel the shoe all along the bottom of your foot? An important detail many people forget is arch support. A lack of arch support can over time cause ankles to turn in, causing discomfort which may eventually radiate upwards towards the knees and hips.
- Stability - Walk around your house or the store. Do you feel your ankles turning in or outwards? Are your feet staying firmly in the shoes?
- Motion Control - Walk quickly and slowly. Do you feel your feet moving in the shoes?
- Wear - before putting on that old pair of sneakers to garden, consider what they felt like when you first bought them. Over time, our weight breaks down the sponge or gel insoles, losing that important support and stability in your shoes. If you notice they are 100% conformed and depressed to your foot's shape, it may be time for a new pair.

Good foot support is important in many ways for gout sufferers. A properly fitted shoe should be comfortably snug, but not tight. Too tight and too loose (flip flops) shoes can irregular circulation. Regular, circulation is essential for flushing of uric acid, buildups of which cause painful gout flare ups. Comfortable shoes are also important for keeping active. An active lifestyle has been proven to lessen the severity of gout flares-but no one wants to exercise if their feet hurt. Talk to your doctor about your gout, and ask if your shoes may be affecting your comfort.

Alcohol and Gout

There are many things that are accepted as "fact" to cause gout flares and attacks. Doctors will tell you, no drinking, no red meats, no asparagus, etc. But how do you know? And how much is too much?

Many people are unwilling to give up their treats and are trying new ways to reduce the occurrences of gout flares. Alcohol in particular seems to be a "vice" of many people. One article quoted a gout sufferer as saying "I will go completely vegetarian before I will give up my wine!" Drinking is an enjoyable social interaction, with billions of people worldwide enjoying booze, whether it is a glass of wine with dinner or having multiple drinks while out dancing.

The question is-are you willing to give all of that up? Many patients are saying no. a trick that has worked for many patients is- if you have alcohol. Flush it out! That is, drink a huge amount of water to "rinse" out the uric acid before it has a chance to crystallize in your joints. This has the added benefit of reducing hangovers!

Most patients, who have had gout for many years, have stated that how much you can drink really depends on how good the rest of your diet is. For example, after eating steak, mashed potatoes and macaroni salad, having a couple of beers isn't necessarily a good idea. But having a healthy salad with grilled chicken may work to allow you to have a glass or two of red wine. Many gout sufferers reported "experimenting" with their diet and alcohol consumption in order to determine which approach is best for them. Are you willing to risk having a gout flare in order to have a drink?

From our research, red wine seems to be the best tolerated of the alcohols. Many patients reported no reaction to having a glass of wine with dinner, or even a glass or two on special occasions. Beer on the other hand, seems to be the least tolerated. There may be many reasons for this. Beer tends to be drunk in multiples, whereas a glass of wine is slowly sipped and enjoyed.

Wine though, especially red wine is quite acidic, and some gout sufferers are reporting that this is causing more flares than beer. All of these gout sufferers though, were taking allupurinol or generic Colcrys for their gout. Be sure to talk to your doctor to determine which is right for you.

Gout and Diabetes Mellitus

Gout is a form of arthritis common among diabetics. Gout and diabetes mellitus are closely related, although they are different medical conditions. Having one condition can increase the risk of developing the other.

- Exercise regularly. Regular exercise will help increase blood flow to the limbs, lower your uric acid level and therefore lessen your chance of developing gout. Experts recommend 30 minutes of moderate activity, at least five days a week.

- Lose weight. According to National Institute of Health, a person's body mass index (BMI) is between 20-24 is normal, over 25 is considered overweight, and greater than 30 is considered obese. The waist size becomes very important if a BMI is over 25. Keep your waist size below 40 inches if you are a man and 35 inches if you are a woman.

- Quit smoking. It will provide you with relief from painful symptoms and improve the blood circulation.

- Skip the alcohol. Eliminate beer or hard liquor will lower the risk of diabetes and cut the risk of developing gout.

Gout and diabetes can also be well controlled with certain medications. You doctor may prescribe generic Colcrys 0.6mg or generic Actos 15mg to improve the circulation and relieve gout symptoms.

Low lead levels in blood linked to gout risk

New study revealed that relatively low level of blood lead may be linked to an increased risk of developing gout, a painful form of arthritis.

Researchers have found that heavy lead exposure is linked to the condition - a form of arthritis in which the joint periodically become swollen, red and hot. Gout most often affects the big toe, though it can also strike the feet, ankles, knees, hands and wrists. Obesity, high blood pressure, diabetes, smoking, certain foods will also boost the gout risk.

The recent research has suggested that low levels of lead which is within the safe range in gout also case gout. In adults, 25 microgram r lesser lead per deciliter of blood is considered safe.

"There is no level of lead exposure which can be termed as 'safe' or 'acceptable'." Said study leader Dr. Eswar Krishnan from Stanford University School of Medicine.

More than 6,100 Americans age 40 and up had their blood lead levels measured from a periodic federal health survey, and were interviewed about their health and lifestyle.

People with highest blood lead levels had more than a threefold greater risk of gout compared with adults with the lowest levels.

People with supposedly safe blood levels of head had a higher prevalence of gout and hyperuricemia than adults who have the lowest lead levels.

Tips for Travelling with Gout

Gout is a form of arthritis caused by elevated levels of uric acid (hyperuricemia) in the bloodstream. Gout happens in men after the age of 50 and women after menopause.

Travel is an example of an activity that may increase the risk of gout. It not only increases stress, but eating and drinking habits may change. Before travelling, discuss with your doctor about preventive measures. The doctor may describe prednidone tablets to be taken immediately at the first sign of a gout acute attack. In most cases, this stops the episode.

Medicines to Take

Could improper footwear be contributing to increased pain in gout patients? Gout causes severe pain and swelling in the joints with the majority of cases affecting the feet. Unfortunately this inflammation can lead to misshapen joints making it difficult to find shoes that feel good. Often gout patients will resort to sandals, flip-flops, moccasins and ill fitting sneakers in an attempt to find comfort. These poor footwear choices may actually cause an increase in gout pain as they lack good support and stability.

A recent study from the University of Oxford in Auckland, New Zealand found that gout patients who chose these types of footwear experienced moderate to severe foot pain, impairment and disability compared with those who made good footwear choices. More than half of the patients in the study wore footwear that was 12 months or older and showed excessive wear patterns.

Better choices for footwear would include, walking shoes, athletic sneakers and oxfords. These choices have better cushioning, support and offer greater stability causing less pain. Look for shoes that have a wide toe box (wide space in the toe area) and removable insoles. Custom orthotics or standard orthotics that mold to the feet can reduce pressure and increase comfort. Buying new shoes at least once a year is also beneficial because even though the shoe may look like it is in good condition, it may have lost some of its conditioning and support.

Choosing better footwear can make a difference in gout pain. Prescription Colcrys can also help by blocking inflammation and relieving a gout attack, usually within 12-24 hours. Combine this with a trip to the shoe store and you could be walking without pain before you know it.

Gout: Not Just for Men

Gout, a form of arthritis that seems to be more common in men than in women, actually does not discriminate according to gender. Women can suffer from this painful condition, too, especially after menopause. Some studies even suggest that there is a definite increase in women with gout, particularly among the aging female population. The exact reason for the rise of gout among women remains uncertain. An Increase in age, obesity, alcohol consumption, hypertension, and diuretic use have traditionally been associated with the risk of developing gout among women and men.

Some facts surrounding the probability of developing gout show consistency. Men between the ages of 40 and 50 appear to be the top candidates. Women rarely show symptoms before menopause. Children and young adults are even less likely to be stricken with this painful disease. Heredity influences whether a person develops gout. Having a genetic predisposition accounts for about 18% of the cases.

As the number of women who have gout increases, there are other complications that can also arise. For instance, women with gout may be 39% more likely to have a heart attack according to some studies. However, the rise in the risk of heart attack for men with gout is far less.

Some people who have chronically high blood levels of urate (commonly referred to as uric acid) will go on to develop gout, but not everyone. In fact, up to roughly 2/3 of the population who have higher levels of urate do not become victims of gout. Why? No one really knows for sure. Could women have less tolerance for higher levels of uric acid since before menopause their levels are generally much lower? Once again, the jury is out on this one.

One thing that we do know is that we have a larger aging population than in the past, and it continues to grow. The number of women who develop gout has doubled. Women need to consult with their doctors if they think that they are experiencing the pain of undiagnosed gout. Because it has been traditionally associated with men and aging, often women are not diagnosed as early as they could have been, postponing treatment. Colcrys 0.6mg tablets may be prescribed for prevention and treatment of gout.

Hot Water for Gout Pain Relief

Gout can be painful. When the build up of uric acid in the bloodstream begins to crystallize and settles in your joints, discomfort follows. Anyone who has suffered from flareups or gout attacks usually welcomes any possible relief from the pain that they can find. Applying heat may help to break up those crystals. When inflammation is reduced the discomfort of an attack subsides. Adding a little Epsom salts and some ibuprofen to the treatment as well can make you even more comfortable.

Running a hot bath or swimming in a heated pool are two great ways to enjoy the benefits of warm water. Test the water first for comfort level. The last thing that you want is to get burned. Also as an additional precaution, diabetics or those with high blood pressure might consider using a hot cloth compress as sitting in a hot bath is not recommended for those conditions. Don't rush the treatment. Try to stay in the water for at least 15 minutes, longer is even better. As you relax in the hot water, the heat dilates your blood vessels, helping to improve circulation.

Epsom salts has magnesium in it. By adding two cups of it to hot water, it may enhance the benefits of the treatment. If you can manage a routine of hot water treatments twice daily, it is even more likely that your pain will be reduced. Taking two Ibuprofen before your bath for extra pain relief is also helpful. Try a soothing foot bath on occasion. Add a third of a cup of ground ginger to the water. You may notice that you start to sweat, but that may also help to eliminate excess uric acid from your system. Once again, don't rush the process. Set aside at least thirty minutes for the foot bath.

Incorporating light stretching before and after the hot bath can be helpful. In fact, stretching, overall, is a good practice to get into regardless of whether you suffer from gout or not. Nothing too vigorous is necessary, but stretching with gout is an ideal way to assist in decreasing the uric acid in the body while increasing flexibility.

In the end, water may be your best friend. Increasing your daily consumption of water is highly recommended. It has been thought that dehydration is a possible contributor to gout. Drinking plenty of water may or using certain medication treatments such as Colchicine 0.5mg can reduce the chances of a gout attack.

The Consequences of Not Treating Gout

Gout cannot be cured. If you have gout, the previous statement is disheartening. However, the good news is that the disease is highly treatable. A form of inflammatory arthritis, gout attacks are painful. Receiving a proper diagnosis in order to begin treating the disease as soon as possible is critical. Some people may develop more serious problems without proper management.

What does your future with gout look like?

- An attack, although painful and even debilitating will most likely begin to subside in a week or two, even without treatment.
- After the first attack, many people may not have another attack for a long period of time.
- Symptoms could eventually return after a period of months or even years.
- Left untreated, the risk of having more attacks is 62% during the first year and increases as time passes. By year three, the risk factor is as high as 84%.

What are the possible complications?

Besides a much higher risk of the return of painful symptoms, when gout is left untreated a person can also develop tophi. Hard nodular masses of uric acid crystals begin to form. A tophus can appear on a finger, toe, ankle, elbow joints, and even on the ears. Most often, numerous little lumps develop. Over time, some may grow to be much larger. Tophi occurs in about 50% of gout cases after 10 years and the risk increases to 72% after 20 years. When is CT Scanning a Good Option for Diagnosing Gout?

Rheumatoid arthritis and osteomyelitis are two diseases that can also mimic the symptoms of gout, thereby contributing to the confusion.

Duel energy computed tomography is a type of CT scan which can replace the traditional method of drawing fluid from the joints. Unfortunately, CT scans are considerably more expensive than the normal tests conducted. Also, there still remains a number of patients for whom the scans do not work. This method of scanning had already been proven to be highly accurate at spotting kidney stones. It was inevitable, perhaps, that testing would be conducted to determine whether it would work as effectively for identifying uric crystals.

At the end of the day, the fact remains that living with the daily pain of chronic gout without a proper diagnosis can be very frustrating. For those cases, in particular, the possibility of CT scanning holds great promise.

Why are Americans at a Higher Risk for Gout?

Gout, a kissing cousin of arthritis, usually attacks the big toe but can spread to the insteps, ankles, heels, knees, wrists, fingers and elbows. If a person suffers from a buildup of uric acid, it may begin to form painful crystals in the joints. Inflammation follows. Occasionally, even kidney stones are formed from uric acid crystals.

Statistically, it appears that gout and arthritis are less likely in countries where there is a smaller amount of meat consumed. Countries like New Zealand, China, Japan and Africa begin to see a rise in cases of gout and arthritis when a Western diet becomes more popular.

Unfortunately, gout is on the rise in the United States. But is there something that we can start doing now to prevent gout? Are there natural substances that when consumed may help to stave off gout in the first place?

A few suggestions

There are a number of alternative remedies that many believe bring relief to symptoms of gout. Although most of the information surrounding these natural adjuncts is anecdotal in nature, all of the touted remedies are good for you whether you have gout or not.

For example, taking several tablespoons of apple cider vinegar, preferably low processed, combined with a little baking soda has been reported to relieve gout symptoms.

Bromelane which is found in pineapple and can be purchased in supplemental forms has beneficial micronutrients and enzymes. Want to go all natural? Juice a whole pineapple. Your body can use this power-packed drink.

Cherry extract and beet juice are both on the list of helpful products. Beet juice has other benefits including helping the kidneys and relieving constipation.

Of course, there are also the obvious lifestyle changes such as losing weight, daily exercise routine and an overall healthy diet which may go a long way towards preventing gout. Why not be proactive and give some of these wholesome possibilities a try?

Can Vitamin C Lower Uric Acid Levels?

The benefits of vitamin C in the diet are undisputed. Vitamin C, a water-soluble vitamin, strengthens the immune-system and protects against infection. It can help to heal wounds. Additionally, vitamin C, also known as ascorbic acid, can provide protective antioxidants which help keep our cells from being damaged by free radicals. For some time now, vitamin C has been touted as a means to lower uric acid in patients diagnosed with gout, but is that claim true?

While vitamin C may help to protect against developing gout in the first place, once diagnosed it does not seem to hold such healing powers. Gout is an inflammatory form of arthritis that causes excruciating pain and swelling in the joints. This is triggered by the crystallization of uric acid. Lowering the uric acid levels are highly recommended in order to alleviate symptoms and get back to a normal life.

The medical community reports that the management of gout requires treatment with medications. In order to lower urate levels, vitamin C does not show significant results even when paired with medication.

Why do some patients fail to lower their urate levels?

The most common reasons patients are not able to lower urate levels are not taking medications regularly or receiving dosages that are inadequate.
