Gout Sufferers - Are your shoes helping you?

Researchers based out of AUT University in Auckland, New Zealand have discovered another environmental factor leading to gout flares- your footwear. "We found that gout patients in our study often wore improper footwear and experienced moderate to severe foot pain, impairment and disability," explains Professor Keith Rome, who led the research team.

Just over half of the subjects wore proper footwear, which includes walking shoes, athletic shoes ("sneakers") and loafers. All of these shoes have a solid foot bed, for good arch, ankle and knee support, and are close-toed. However, 42% of gout sufferers wore unsuitable shoes such as flip-flops, sandals and moccasins. Another factor of good shoes is their age. Some of the sufferers were wearing the right shoes, but researchers were finding them to be over 12 months old or older. The benefits of good shoes steadily decrease as the get older and more worn.

When choosing which shoes to wear or buy, consider these factors once you slip a pair on:

- Cushioning - Do you feel your feet lightly depress the insole? Do your toes have wiggle room?
- Support - Can you feel the shoe all along the bottom of your foot? An important detail many people forget is arch support. A lack of arch support can over time cause ankles to turn in, causing discomfort which may eventually radiate upwards towards the knees and hips.
- Stability - Walk around your house or the store. Do you feel your ankles turning in or outwards? Are your feet staying firmly in the shoes?
- Motion Control - Walk quickly and slowly. Do you feel your feet moving in the shoes?
- Wear - before putting on that old pair of sneakers to garden, consider what they felt like when you first bought them. Over time, our weight breaks down the sponge or gel insoles, losing that important support and stability in your shoes. If you notice they are 100% conformed and depressed to your foot's shape, it may be time for a new pair.

Good foot support is important in many ways for gout sufferers. A properly fitted shoe should be comfortably snug, but not tight. Too tight and too loose (flip flops) shoes can irregular circulation. Gout flares can be both painful and debilitating. Throbbing pain in your knee or foot usually makes your exercise decision for you. But how to start? Your first step should be talking to your doctor. Your doctor can recommend which specific exercises can be safely used on your body. Everyone has gout in different parts of their body, and what works for your friend may have a negative effect on you. Also, remember that if you have a gout flare-up, be sure to stop exercising. You can get back on your schedule once the pain is gone.

"Range of motion" exercises tend to be a core element in a gout exercise regimen. Exercises such as tai chi, band exercises and gentle yoga are great for keeping the joints limber. Researchers in Denmark found that patients on the strict diet for 16 weeks lost radical amounts of weight and reported a huge improvement in their pain levels, a daily reported.

A simple diet, which is very low in calories and high in protein combined with regular exercise, could end the misery of arthritis for millions of sufferers, according to new research. A relatively small weight loss can have a dramatic effect by reducing swelling, relieving pain and even helping prevent further deterioration of the joints.

"Research shows that losing weight, however modest, when combined with exercise, is a panacea at every stage," Professor Alan Silman, medical director of Arthritis Research UK, said.

"A healthy weight reduces the risk of developing the disease, relieves existing symptoms and helps to prevent further deterioration. Weight loss and exercise has been shown to achieve the same level of symptom relief as joint replacement surgery."

This is good news for gout sufferers, as a low calorie diet can be also low in purines, which will help to reduce the severity of gout flares they will experience. For the high protein, stick to beans, soy or chicken, as red meats, especially steaks and organ meats, have been known to cause flare ups.

The study participants were also placed on a regimented exercise plan, which helped them to lose the weight while still building muscle. A new type of arthritis therapy has recently been approved in the United Kingdom. A type of 'pen' that uses super-cold temperatures to stop pain signals could provide relief for thousands of arthritis patients. The handheld device contains a tiny needle-like tip that is cooled to minus 20c this is inserted just a few millimetres under the skin against a nerve. The theory is that the cold temperature makes nerves 'hibernate' so that the pain signals are blocked.

The new gadget which is the size and shape of a pen could also work for a wide range of conditions, say the manufacturers, including headaches, arthritis, facial pain and carpal tunnel syndrome. The therapy can be repeated continually in a GP surgery or pain clinic. Cold temperatures are increasingly being employed by doctors to treat a range of pain conditions.

The treatment is called cryotherapy, and is thought to work by blocking the ability of nerve fibres to transport signals. The conduction of signals along our nerves involves the coordination of a number of complex chemical reactions. It's thought that cold temperatures can slow the rate of these reactions, and lead the nerves to become 'quiet' and stop sending signals. This is partly why our hands and feet go numb in cold temperatures (the cold also causes blood vessels in the extremities to shrink, further adding to the numb sensation).

The cryotherapy pen uses cold liquid nitrogen to chill the tip. When the pen is switched on, the cold liquid inside the device is put under high pressure, which causes it to turn into a super-cold gas. This gas then cools the metal tip. A doctor places the tip which is the same thickness as a needle into the patient's skin, which cools the surrounding tissue in a matter of seconds. Sensors in the device monitor the temperature of the skin throughout the treatment, to ensure the tip is cold enough to send the nerves into 'hibernation', but not so cold that it could damage the tissue.

Each treatment session takes around 30 to 45 minutes. It can be repeated as often as required, and the researchers say that the effects last for differing times depending upon the condition that is being treated. Clinical trials are still ongoing, but early data from more than 100 patients already shows that the system can be effective. As well as investigating its use for pain disorders, researchers are also looking into whether the device could work on nerves that control movement. They hope that this will enable it to be used for conditions that cause upper limb spasticity such as stroke, multiple sclerosis or Parkinson's. These disorders cause nerve damage, which triggers uncontrolled muscle activity such as clenching, twitching or spasm.

The cold pen has the potential to deactivate the malfunctioning nerves and block the uncontrolled muscle activity. Clint Carnell, chief executive of U.S.-based Myoscience, which developed the device, said: 'We currently have studies looking at its effectiveness for pain and for movement disorders and if all goes well we would hope to launch in the UK in 2013.'

Until the cryotherapy pen is approved around the world, arthritis sufferers, especially those with gout, should ask their doctors about colchicine 0.6mg, a gout treatment that has been available for hundreds of years.
